Some symptoms take longer than 90 days to fully resolve, and a few never resolve without addressing other factors. Honest expectations and consistent monitoring are what separate a good TRT outcome from a disappointing one.

This is a clinician’s-eye view of what the first 90 days actually look like, what your provider should be doing, and where the real risks live.

At a minimum, your baseline should include:

  • Total and free testosterone (free is what your tissues actually use)
  • Estradiol (E2, sensitive assay), SHBG, LH, FSH, prolactin
  • Complete blood count (CBC) with hematocrit
  • Comprehensive metabolic panel and a lipid panel
  • PSA if you are 40 or older, or earlier if there is family history
  • Thyroid panel and vitamin D, because they often shape how you feel as much as testosterone does

Phase 1: The First Two Weeks

The first two weeks are where expectations have to be calibrated. The most common pattern: a quiet, subtle lift in morning energy and slightly better sleep. Patients often describe it as “the floor coming up” rather than a peak. Some men feel nothing at all in week one, and that is normal.

What you should NOT expect in the first two weeks:

  • Visible strength gains in the gym
  • Body composition changes you can see in the mirror
  • A stable, reliable libido (it may flicker on and off)
  • Resolution of long-standing mood symptoms

Side effects to flag to your provider in this window: noticeable water retention, breast tenderness, mood swings, persistent insomnia, or injection-site reactions. None of these are emergencies on their own. All of them are signals worth reviewing.

Phase 2: Weeks Three Through Six

This is when most patients say something has clearly changed. Sleep gets deeper. Mood stabilizes. Libido becomes more consistent rather than coming and going. Patients who train start to notice they recover faster between workouts, even before the weight on the bar moves.

Body composition begins to shift in this window. Lean mass starts to build, and fat mass usually starts to come down, especially around the midsection. The change is rarely dramatic by week six. It is real, but it is more visible on a DEXA than in a mirror.

If you are not seeing changes by week six, the protocol may need adjusting. Common reasons it stalls: dose is too low, frequency is too spread out, estradiol has climbed too high, or sleep and nutrition are not supporting the work. This is exactly the kind of thing the next lab recheck is for.

Phase 3: Weeks Seven Through Thirteen

By the back half of the 90 days, the gains start to compound. Strength climbs, endurance improves, and mental baseline tends to settle at a higher set point: less reactive, more durable, more like yourself.

Around day 60 to 90, we recheck labs to confirm the protocol is doing what it should. We look at:

  • Total and free testosterone on the trough day (just before your next dose) to confirm we are inside the right therapeutic window
  • Estradiol (E2), to make sure aromatization is balanced and you are not running too high or too low
  • Hematocrit, because TRT thickens the blood and we want it controlled
  • PSA, especially in men 40 and over
  • Symptoms and side effects in your own words

This is the visit where the protocol gets refined: a small dose change, a frequency change, the addition or removal of an adjunct. The first 90 days are not the destination. They are how we calibrate.

What We Watch For (And Why)

TRT is a medical therapy. It has real risks if it is not monitored. The big four we track on every recheck:

  • Estradiol (E2). Testosterone naturally converts to estrogen. Some is essential for libido, mood, joints, and bone. Too much causes water retention, sensitive nipples, and emotional volatility. Too little (often from over-medicating against it) tanks libido and joints. The goal is balance, not zero.
  • Hematocrit. TRT raises red blood cell production. If hematocrit climbs too high, blood gets thicker, which increases cardiovascular risk. Managed with hydration, dose adjustment, more frequent smaller doses, or, if needed, a therapeutic blood donation.
  • PSA. TRT does not cause prostate cancer, but it can accelerate something already present. We track PSA trends, not just absolute values, especially in men 40 and older.
  • Free vs. total testosterone. Total testosterone is the headline number. Free testosterone is what your tissues can actually use. SHBG and albumin determine the relationship between the two. We dose to the free number, not just the total.

Fertility, HCG, and Gonadorelin

When you start TRT, your body senses there is plenty of testosterone in the system, so it tells your testicles to slow down or stop their own production. For men who are done having kids, that is usually not a concern. For men who want children in the future, it is one of the most important conversations we have at day zero.

This is where peptide adjuncts come in. Medications like HCG (human chorionic gonadotropin) and gonadorelin signal the testicles to keep working alongside the TRT. They help preserve testicular size, support endogenous testosterone production, and protect fertility for many (not all) men. They are not interchangeable, and they are not for everyone. Tracking Body Composition the Right Way

The scale is a blunt instrument for TRT. Lean mass weighs more than fat by volume, so the number can stall or even climb a little while your body is unmistakably leaner. We prefer DEXA scans at baseline and again around 90 days because they separate lean mass, fat mass, and bone density.

If DEXA is not available, a tape measure at the waist, a few honest photos in the same lighting, and grip-strength or barbell-strength benchmarks tell you most of what you need to know. What Derails Outcomes

Most TRT disappointments trace back to a short list of preventable problems:

  • Under-dosing. A starting dose that is too low or too infrequent leaves you stuck in the middle of the range with most of the side effects and few of the benefits.
  • Poor monitoring. Skipping the 90-day recheck, or never checking estradiol and hematocrit, is how the avoidable risks become real ones.
  • Gray-market sourcing. Testosterone bought online or from non-medical sources is not regulated for purity or dose accuracy. The cost savings disappear the moment something goes wrong.
  • Ignoring the basics. TRT works better when sleep, training, alcohol intake, and stress are addressed alongside it. Hormones do not override lifestyle.

Who This Is Right For, and Who It Is Not

A medically supervised TRT protocol is appropriate for men with documented low testosterone and symptoms that match. It is not a shortcut for normal aging, a substitute for sleep and training, or a tool to push physiologic testosterone into supraphysiologic ranges. Is TRT safe long-term?

The risks (elevated hematocrit, estrogen imbalance, untreated prostate issues, cardiovascular stress) are real, and all of them are manageable with proper labs and dose adjustments. Unsupervised TRT is where the real risks live.

Can I work out hard while starting TRT?

Yes, and we encourage it. Strength training in particular helps TRT do its job on body composition. Just expect that strength gains will trail the hormonal changes by a few weeks. The first month is about consistency, not personal records.
